Requirements :
1. Original/ Photocopy of Form138 (School Report Card)
2. Certificate of Good Moral Character
3. Birth Certificate ( photocopy )
4. ESC Certificate ( original / photocopy )
5. Clearance from school last attended / Certificate of Transfer Credentials (formerly referred to as “Honorable Dismissal”) it is a document certifying that a student has no pending accountabilities and is eligible for transfer.
6. 2 Copies of 2x2 picture
The Pamantasan ng Lungsod ng Marikina (PLMar) is a government-funded university in Marikina City offering free and high quality education for the students entering this university.
The Pamantasan ng Lungsod ng Marikina Senior High School offers academic tracks for students:
* ABM Strand (Accountancy, Business and Management)
* HUMSS Strand (Humanities and Social Sciences)
* STEM Strand (Science, Technology, Engineering and Mathematics)
You may order your transcripts by filling out an online request form through our website (Click the 'Request Form' located at the homepage. For an easier access, look at the quick links provided at the right side of our webpage.)
Upon request of transcript, this should take about 2-3 working days depending on the day of request. The office of the registrar suggests that requests needed for urgency shall be requested sooner to avoid sudden influx of requests in the registrar.

Certifications and other Forms/Letters may be requested, just indicate the purpose for request and simply fill out the e-request form. The request for documents shall be made available after 2-3 working days.
